Our mission is to empower individuals by fostering independence through the development of social skills, life skills, advocacy, training, and education.
We are committed to providing personalized support that equips each client with the tools necessary to lead a safe, fulfilling, and healthy life with the greatest level of autonomy possible.
Guided by faith and a spirit of service, we strive to uplift our community and support those in need through t.e.a.m.s, (trust, encouragement,acceptance, motivation, and safety.Our goal is to help individuals become confident self-advocates, cultivate meaningful relationships, pursue purposeful careers, and experience happiness and success in their daily lives.

Program Specifications
S.L.A.T.E. operates through two legally separate but mission-aligned entities, each serving a distinct role:
Slate Family Services, LLC is a for-profit organization providing non-clinical, skills-based services and supports, including adult habilitative services and contracted programs.
Slate Family Services Foundation is a nonprofit organization focused on charitable, educational, and prevention-based programs, including youth development, family education, and community inclusion initiatives.
Each entity maintains separate governance, funding, and compliance requirements, while working collaboratively to support individuals, families, and communities.
